Formaloo 5.0 brings the Intake Agent, Formaloo Decisions One, an agent that builds and runs whole operations from one sentence, and Formaloo inside Claude and ChatGPT. A forward-deployed team builds the system with you.
TORONTO, October 15, 2026. Formaloo today launched Formaloo 5.0, the first full release of Formaloo OI (Operations Intelligence). Formaloo OI turns the processes an organization runs every day, like applications, approvals, intake, and onboarding, into AI systems that collect information, decide what it means, and act on it. People stay in charge of every decision that matters.
In August, we said Formaloo is no longer a form builder. Today, that direction ships. Formaloo 5.0 brings a new brand, a new website, a new dashboard, and an army of prebuilt agents for the 40,000+ teams in 100+ countries that run on Formaloo.
"Most companies don't need another AI tool. They need the work around their processes to disappear," said Farokh Shahabi, co-founder and CEO of Formaloo. "Formaloo OI starts from the outcome a team cares about, redesigns the process around it, and then runs it. Routine cases move on their own. The hard ones reach a person with everything they need to decide."
The problem was never a lack of AI
Most of the talk about enterprise AI has been about models, copilots, and chatbots. For most organizations, the harder problem is operational. Information sits in emails, spreadsheets, and a CRM. One process crosses four teams. Approvals wait in inboxes. And people spend their days checking whether the last step happened.
Give each step its own AI assistant, and the process gets a little faster. Formaloo OI asks a different question first: does this step need to exist at all?
Take an insurance or workers' compensation claim. Today, someone reads the submission, checks the policy, asks for missing documents, assesses the risk, updates three systems, and routes the payment for approval. With Formaloo OI, the claim becomes one continuous process. The system reads it, pulls in the customer and policy context, asks for what's missing, applies the organization's rules, and flags anything unusual. Routine claims move through. Complex or high-risk ones reach a specialist with the analysis already done.
It isn't a faster version of the old process. It's a different way of operating.
Business intelligence tells you what happened. Operations intelligence makes the right thing happen.
The five headline announcements
1. The Intake Agent: the next generation of forms
Information rarely arrives through a form. It comes by email, on WhatsApp, in a phone call, or it's already sitting in a CRM. The Intake Agent takes it from every channel, understands it, and turns it into one clean, structured record. It matches each person to the record they already have instead of creating duplicates. It follows up until it has everything, with different rules for different people. Then it sends the finished record to your system of record, whether that's a CRM, an ERP, or any service that takes a webhook.
The Intake Agent collects, understands, and acts on its own. That's why it sits at the core of Formaloo OI.
2. Formaloo Decisions One: AI that makes the call
Large language models write well. Operations run on decisions: approve or escalate, urgent or not, which team, what score. Formaloo Decisions One is a decision model built into Formaloo for exactly that. It picks the option, gives the score, answers yes or no, and says how confident it is.
There's no schema to write and no code. Describe how your best manager makes the call, in plain words, and Formaloo plans it. You see the plan, run it across every record, and watch each decision it makes. Every decision feeds a context graph of your operation. When your team overrides a decision, or an outcome turns out differently, Formaloo notices and suggests how the process should change. Nothing changes until a person approves it.
3. Describe it, and Formaloo OI builds it
Describe a goal in one sentence. Formaloo OI asks the follow-up questions a good consultant would ask: who signs off, which channels to use, what happens at the edges. Then it shows you the whole plan before anything exists. Approve it, and the Form Creation Agent, Logic Agent, and Design Agent build the forms, routing, emails, PDFs, reminders, and access rules as one connected process. Run it as a test participant before it goes live. When you want a change, say so: "make the reminder friendlier" or "add a second approver."
The agent lives inside the new Formaloo dashboard. There's no AI subscription to bring and no keys to set up.
4. Formaloo Agents: workflows that do the work
Starting with Formaloo 5.0, workflows are agents. Formaloo OI comes with an army of prebuilt agents to run common processes, the Intake Agent among them. Customize any of them, or build your own to do anything your business needs.
5. Formaloo, without opening Formaloo
Formaloo now works inside Claude and ChatGPT, with listings in both app directories. Describe a form or process in the chat, and Formaloo creates it, brands it, wires the logic, and publishes it, with a live preview right in the conversation. The Formaloo MCP, the technology behind it, also connects Cursor and other AI tools. It's free on every plan, with unlimited usage. Developers get the Formaloo CLI too.
Collect: every answer, without the form
- AI-agent campaigns. Send a campaign by email or WhatsApp, and the Intake Agent turns every reply into a structured record. People reply to messages. They ignore survey links.
- NoForm. The Intake Agent and campaigns together mean you never have to build a traditional form, and your audience never has to fill one in, unless either of you wants to.
- Headless Forms. Design the form in Webflow, Framer, React, or plain HTML. Every submission still runs through Formaloo's logic, emails, approvals, and integrations.
- Autofill with Voice. Field staff speak, and the form fills itself in. They review every field before they submit.
- Incoming webhooks. Any external system can send data straight into a Formaloo record, ready for logic to act on.
- New fields. Ranking, Slider, searchable Multi-select Dropdown, Contact Info, a Review step before submit, invisible fields for campaign and tracking data, and multi-field rows that make long forms look short.
- Better portals. Allow sign-ups only from your company's email domain, let people sign in with a magic link, and let them edit a submission until it's approved.
- More ways to embed. Popup embeds, rebuilt embeddable forms, and a mobile preview in the form editor.
Understand: from summaries to decisions
- The Agent Field. Add an agent to any step and describe its job in plain words. It runs every time that step runs. It looks things up on the web, calculates, fills in missing details, updates fields on the record, analyzes uploaded files, and triggers the next actions. Re-run it whenever the context changes.
- The Analyst Agent. Ask questions about your data and get answers, reports, and charts.
- Related Records. Open a record and see its whole story: every related stay, request, or application from every connected table, in one place. It's a CRM view, without building a CRM.
- Calendar and timeline views. See and manage records by date. Drag a booking to a new day, or stretch a project bar to a new deadline.
- A faster table. The new table block lets teams edit records inline, like a spreadsheet.
- Formaloo Research. Some questions are bigger than the data you already have. Formaloo Research, a new Formaloo company, runs large market and product research studies. Our researchers design the study, Formaloo agents reach thousands of people on every channel, and the team turns the analysis into answers leadership can act on, at a fraction of an agency's cost.
Act: agents that run your operations
- One rule, many actions. When an organizer confirms an event, a single rule can create the event, update the guest's profile, send a WhatsApp confirmation, and notify catering.
- Write anywhere. Agents create and update records in any form, count records before they act, and send WhatsApp messages.
- Follow-ups. Write one sentence, like "three days before the event, if the headcount is still missing, WhatsApp the organizer." Formaloo turns it into wait, check, act. The check runs against the record as it stands, so nobody gets chased for something they already sent.
- Teams and tasks. Create departments, and assign work by logic to a person, a team, or an agent. Every item gets a status and a due date, and the activity page shows everything that happened. Formaloo is now a project and task system for operations teams.
- Capacity and reservations. Set limits per option, per session, or per form. The next person moves off the waitlist the moment someone cancels.
- Documents and eSign. Describe a document to ChatGPT or Claude, paste the HTML into Formaloo, and watch it render as a PDF. Every booking then produces its own contract, sends it for signature, and stores it with its record. The rebuilt email and PDF editor lets teams switch between visual and HTML editing.
- One-click updates. Every email and PDF can carry a secure link that opens the exact record. The edit screen now looks and behaves like the form itself.
- AI tools everywhere. The Import Agent recreates an existing form in Formaloo. The Question Generator suggests the right fields, Writing Tools rewrite and proofread, the ID Generator prepares fields for agents and the MCP, and AI Content writes personalized emails, ending pages, and PDFs from each record.
Trust: AI that follows your rules
Healthcare, insurance, education, and public-sector teams ask the same first question about AI: where does our data go, and who can touch it? Formaloo 5.0 answers it in the product. Agents propose, and people approve. Formaloo records every action, and permissions decide what each agent can see and do.
- Google and Anthropic partnerships. Zero data retention, and customer data never trains their models.
- The workspace AI switch. One toggle turns off every AI feature in a workspace.
- Version history. Formaloo saves every change to every form, and you can roll back at any time.
- Automatic anonymization. Personal data anonymizes itself on the schedule you choose.
- Identity and access. Single sign-on with Descope and Azure, tighter workspace permissions, folder access that flows down to every project inside it, reCAPTCHA, and AI-powered anti-phishing protection.
- Your company's Formaloo. Your own sidebar, homepage, and company-wide pinned projects, plus your brand themes and time zone.
- Deploy anywhere. Run Formaloo in our cloud, hosted in the EU by default, in the region you choose, or self-hosted in your own infrastructure, including OpenShift.
A new look, inside and out
Formaloo 5.0 brings a new logo and a new website. On formaloo.com, visitors describe a process in plain words, and the homepage agent shows how to build it, or hands complex processes to our team. Inside the product, the new dashboard puts everything in the right place: apps like email and PDF templates move into the sidebar, a command palette gets you anywhere in a few keystrokes, and a new Integrations page replaces the old app store. The sign-in and MCP authorization screens match the new design.
The team that builds it with you
Software alone rarely fixes a broken process. So Formaloo OI comes with a forward-deployed team that works inside your operation. They map the people, systems, rules, and exceptions involved, then work back from the result you want.
It starts with a free process review. Then comes a blueprint, the deployment, and ongoing optimization. Every engagement begins with a measurable goal, like cutting onboarding from twelve days to two. It's done when that number moves, not when a feature ships.
Organizations like Concordis already work this way, and expand once the first process runs.
"Each workflow saves us 3–7 emails per engagement, streamlining communication and keeping everything organized in one place."
Heather Lloyd, Executive Director, Concordis
Formaloo is now a group of companies built around one platform. Formaloo Platform is the engine. Formaloo OI is the forward-deployed team that redesigns and runs processes on it. Formaloo Research runs research at enterprise scale. And Formaloo BranchOps is our forward-deployed company for hospitality and multi-location operations, where one business can mean hundreds of branches.
Formaloo OI also works with the systems a company already relies on. Your CRM, ERP, and document tools stay the systems of record. Formaloo becomes the layer that collects, decides, and acts across them.
Availability and launch offer
Formaloo 5.0 starts rolling out today, and every workspace gets the full release over the coming weeks. Every customer with an active paid account before November 15 gets free AI credits for a full year, with unlimited usage.
Farokh walks through Formaloo 5.0, with demos of everything above, in a keynote premiering on YouTube.
